This form must be filled out and signed by the player's parent or guardian prior to the player taking part in any BLL activity, including assessments and clinics. We are required by Little League International to have this form in our possession when your player is participating. A new form must be filled out each season.

This form must be filled out by anyone who wishes to volunteer in the League, or to work with players in any capacity. The background check form must be filled out and approved prior to a volunteer taking part in any BLL activity, including assessments and clinics. A new form must be filled out each season.

Ballard Little League expects all volunteers to become members of the league. For a minimal cost ($5), this enables the member to attend meetings and help decide the course of the league. A new form must be renewed each season.
